Methodology

This report analyzes CMS inspection records from the ProPublica Nursing Home Inspect database. A "serious deficiency" is defined as any deficiency with scope/severity rating of J (potential for harm), K (actual harm), or L (immediate jeopardy).

Date Range: January 1, 2022 – May 31, 2026
